Job summary

Opco-Skilled-Management- is seeking a qualified MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator. The role ensures documentation meets Federal, State, and Certification guidelines and coordinates the RAI process for timely MDS completion.

The ideal candidate holds a nursing license (RN/LVN) and has at least 2 years of SNF experience, with strong knowledge of Medicare PPS, Medicaid reimbursement, and OBRA regulations.

Qualifications

• Graduate of an approved Registered Nurse / License Vocational Nurse program and licensed in the state of practice required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement; OBRA regulations and MDS process.

Responsibilities

• Assists the center in assuring adherence to Federal and State regulations and certification.
• Participates in regulatory or certification survey process and correction of deficiencies.
• Reports trends from audits to Quality Assurance Committee.
• Ensures timely completion of the MDS through the interdisciplinary care plan.
• Monitors RAI process tracking, discharge/reentry and Medicaid forms in PointClickCare.
• Follows up with staff to ensure documentation standards are met.
• Completes patient assessments and data collection; interviews staff as needed.
• Determines Assessment Reference Date reflecting care needs and resource use.
• Submits MDSs to the State with proper follow-up on validation errors.
