Job summary

Outfield Healthcare Partners in Clinton, TX, is seeking an MDS Coordinator to ensure timely and accurate completion of the RAI process and care management from admission to discharge in compliance with company policy and federal/state guidelines. Under direction from leadership, you may coordinate information systems operations and education for the clinical department.

The role requires RN licensure, familiarity with MDS 3.0, and strong knowledge of Medicare PPS and Medicaid reimbursement.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of OBRA regulations and MDS.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Works with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required by agencies.
• Ensures the Interdisciplinary team decides on completing or not completing additional MDS assessments based on criteria.
• Assist with coordination of daily stand-up meetings and review of resident care and assessment reference dates.
• Complies with federal and state regulations regarding completion and coordination of the RAI process.
• Monitors MDS and care plan documentation to support MDS coding in medical records.
• Maintains current MDS status per state and federal guidelines and enters data into MDS programs.
• Completes accurate coding of the MDS from chart review and interviews with staff, residents, and families.
• Attends interdisciplinary meetings to gather information and communicate changes.
