Job summary

Outfield Healthcare Partners seeks an experienced MDS Coordinator to lead MDS assessments and care planning from admission through discharge. You will work under the Director of Nursing and VP of Clinical Reimbursement to ensure compliance with Federal, State, and Certification guidelines.

This role requires RN licensure, SNF experience, and strong knowledge of MDS 3.0, OBRA, and PPS. Relocation to New Mexico is required, with multiple facility options and sign-on considerations.

Qualifications

• RN licensed in state of practice.
• Minimum 2 years in a Skilled Nursing Facility preferred.
• Knowledge of OBRA, MDS, and Medicaid PPS.
• Experience with MDS 3.0 preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the interdiscipl. team.
• Ensure timely completion of RAI process per guidelines.
• Monitor MDS and care plan documentation in medical records.
• Maintain current MDS status per state and federal rules.
• Schedule resident care conferences and communicate outcomes.
• Train staff in RAI, PPS, and clinical computer systems.
