Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator for NM facilities. You will manage MDS and RAI processes from admission to discharge, ensuring compliance with Federal, State, and Certification guidelines. Collaboration with the Director of Nursing and VP of Clinical Reimbursement is essential.

The role requires RN licensure, knowledge of Case-Mix and PPS, and experience with MDS 3.0. Relocation to New Mexico is required, with multiple facility locations to choose from and

Qualifications

• RN license in practicing state required.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Understanding of OBRA regulations and Minimum Data Set.
• Experience with MDS 3.0 preferred.

Responsibilities

• Works with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decides on MDS completion based on clinical criteria per the MDS User’s Manual.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Ensure compliance with federal and state regulations for MDS process.
• Monitor MDS and care plan documentation in medical records to support coding.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data accurately into computerized MDS programs.
