Job summary

Outfield Healthcare Partners seeks an experienced MDS Coordinator to ensure timely and accurate completion of the RAI and care management processes from admission to discharge in line with company policy and federal/state guidelines.

Under the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and education for the clinical department across New Mexico facilities listed below.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required by governing agencies.
• Ensure decisions on MDS activities follow clinical criteria per the RAI User’s Manual.
• Assist with coordination of daily stand-up meetings and review resident care against assessment reference dates.
• Comply with federal and state regulations regarding MDS process completion and coordination.
• Monitor and maintain MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status per state and federal guidelines and ensure timely data entry into MDS systems.
