Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator to oversee timely and accurate completion of the RAI/MDS process from admission to discharge, aligned with policy and federal/state guidelines. You will work under the Director of Nursing and VP of Clinical Reimbursement to coordinate information systems and education for the clinical team.

The role requires an RN license, 2+ years in a Skilled Nursing Facility, and strong knowledge of MDS 3.0, PPS, and Medicaid

Qualifications

• RN licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process, OBRA regulations and MDS.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing or not completing additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Ensure compliance with federal/state regulations for MDS processes and documentation.
• Maintain MDS status and data entry in appropriate systems.
• Attend interdisciplinary meetings to communicate changes and update records.
• Assist with identification of significant changes and reflect them in MDS and care plans.
