Job summary

Outfield Healthcare Partners seeks an experienced MDS Coordinator to manage timely and accurate RAI and care management processes from admission to discharge, aligning with policy, guidelines, and regulatory standards.

Under direction from Nursing leadership, you may coordinate information systems operations and education for the clinical team, ensuring compliant MDS coding and care planning.

Qualifications

• Minimum 2 years nursing experience in a Skilled Nursing Facility is preferred.
• Excellent knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement as required.
• Understanding of Quality Indicator process and OBRA regulations.
• Knowledge of care planning processes.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Assist in determining whether to complete additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Comply with federal and state regulations for the RAI process.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status per state and federal guidelines.
• Ensure accurate data entry of resident information into MDS systems.
• Complete MDS coding from chart review, observation, and interviews.
• Attend interdisciplinary meetings and other meetings to gather and communicate updates.
• Assist DON with significant changes and physician orders reflected in MDS and care plans.
• Prepare scheduling and notices for care planning conferences and communicate outcomes.
• Update knowledge on data entry and computer technology; submit documentation to state databases.
