Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator to manage the RAI process from admission to discharge across multiple New Mexico facilities. You will work with an interdisciplinary team to ensure timely, accurate MDS coding and care planning in compliance with state and federal guidelines.

Relocation to New Mexico is required; pick a facility location from Alamogordo, Roswell, Gallup, Farmington, Taos, Las Cruces, Grants.

Qualifications

• RN licensed in the practicing state.
• 2+ years nursing in a Skilled Nursing Facility preferred.
• Knowledge of MDS 3.0, RAI process and Medicaid reimbursement.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the interdisciplinary team.
• Ensure compliance with federal/state regulations for the RAI process.
• Maintain MDS status and accurate data entry in the system.
• Prepare resident care planning conferences and communicate outcomes.
