Job summary

SNF Staffing Solutions is seeking an experienced MDS Coordinator to join our team and relocate to New Mexico. You'll manage RAI, care plans, and MDS coding across multiple NM facilities, ensuring compliance with federal and state guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you will lead data entry, documentation, and education for the clinical department, with a focus on accurate, timely reporting and collaboration with the interdisciplinary team.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on MDS, additional assessments are based on clinical criteria per the RAI Manual.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Comply with federal and state regulations for MDS process completion and coordination.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status and accurate data entry in MDS software.
• Code MDS information from medical records, observations, and interviews.
• Attend interdisciplinary meetings to communicate changes and update records.
• Assist DON with significant changes, physician orders, and verbal reports to reflect changes.
