Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator to relocate to New Mexico and support multiple facilities. You will oversee the RAI and care management processes from admission to discharge in line with policy and regulatory requirements.

Under guidance from nursing leadership, you may coordinate clinical information systems operations and education for the department, ensuring accuracy, timely reporting, and high-quality patient care.

Qualifications

• RN license in state of practice required.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process and OBRA regulations.
• Knowledge of the care planning process and MDS 3.0 is preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to schedule MDS assessments and care plan reviews.
• Decide on whether to complete additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings and review resident care and assessment dates.
• Ensure compliance with federal and state regulations for RAI processes.
• Maintain MDS status and input data into appropriate systems.
• Prepare and communicate scheduling and outcomes to staff, residents, and families.
