Job summary

Outfield Healthcare Partners in New Mexico seeks a seasoned MDS Coordinator to manage the RAI and care‑planning processes from admission through discharge, aligning with policy and federal/state guidelines.

Under direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate data entry, assist with education, and ensure timely, accurate MDS coding and documentation across facilities in several NM locations.

Qualifications

• Must be a graduate of an approved RN program and licensed in the state.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility.
• Solid knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and MDS processes.
• Experience with MDS 3.0 preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to schedule MDS assessments and care plan reviews.
• Ensure decisions on MDS completion based on clinical criteria per RAI Manual.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Comply with federal and state regulations for RAI processes.
• Monitor MDS and care plan documentation in the medical record.
