Job summary

Trilogy Health Services is seeking a dedicated MDS Coordinator (LPN, RN) to oversee resident assessments and care planning, ensuring compliance with federal and state regulations for Medicare/Medicaid reimbursement. Based in Liberty Township, OH, you will ensure accurate MDS data and timely reporting.

The role requires a valid LPN/RN license and 3–5 years in MDS/assessment roles, with CPR certification. You’ll join a supportive team within Trilogy’s Thrive-focused benefits culture.

Qualifications

• Must hold a current, valid LPN or RN license.
• Experience of 3–5 years in MDS or assessment roles in long-term care, senior living or similar settings preferred.
• CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents’ status and triggers, including CAA implementation.
• Evaluate residents’ condition to determine need for special assessments or admission amendments.
• Prepare and electronically transmit timely reports to national Medicare/Medicaid databases.
• Develop a written plan of care for each resident identifying problems/needs and goals.
• Provide information to residents/families on Medicare/Medicaid and financial assistance options.
• Ensure MDS notes accurately reflect services and resident response.
• Assist with completing the care plan portion of the discharge plan and implement committee recommendations.
