Job summary

Trilogy Health Services in Ohio seeks an experienced MDS Coordinator (LPN/RN) to oversee resident assessments and care planning, ensuring compliance with federal and state regulations related to Medicare/Medicaid reimbursement.

You will conduct MDS assessments, evaluate resident status, and develop comprehensive care plans while providing information on Medicare/Medicaid and financial assistance. This on-site role requires CPR and a current license.

Qualifications

• Experience in MDS or assessment in senior residential care or long-term care preferred.
• Current CPR certification required.
• Must hold a valid state LPN or RN license.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents' status and triggers.
• Evaluate residents' condition to determine need for additional assessments or plan amendments.
• Prepare and electronically transmit timely reports to Medicare/Medicaid databases.
• Develop a written plan of care for each resident with goals and problems identified.
• Provide information to residents/families on Medicare/Medicaid and financial assistance programs.
• Ensure MDS notes describe services provided and resident response.
• Assist with care plan portion of discharge planning and implement committee recommendations.
