Trilogy Health Services, LLC in Englewood, Ohio is seeking a qualified MDS Coordinator (LPN or RN) to oversee resident assessments and care planning, ensuring compliance with federal and state regulations for Medicare/Medicaid reimbursement. You will conduct MDS assessments, support care plans, coordinate with care teams, and assist residents and families with Medicare/Medicaid options.

A current LPN/RN license and CPR certification, with 3–5 years in an MDS or assessment role in long-term care,

Qualifications

• Must have a current, valid LPN or RN license.
• Three to five years’ experience in MDS/assessment roles in long-term care.
• Current CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents’ status and triggers.
• Evaluate condition and medical data to determine need for additional assessments or care plan changes.
• Prepare and transmit timely reports to Medicare/Medicaid databases.
• Develop preliminary and comprehensive care plans for each resident.
• Provide information to residents/families on Medicare/Medicaid options.
• Ensure MDS notes reflect services provided and resident response.
• Assist with care plan portions of discharge plans and participate in committee recommendations.
