Qualifications

Responsibilities

• Conduct Minimum Data Set (MDS) assessments for residents.
• Prepare and transmit reports to Medicare/Medicaid databases.
• Develop written care plans for each resident.
• Provide residents and families information on Medicare/Medicaid.
• Ensure informative MDS notes reflect services provided.
