Job summary

The MDS Coordinator (LPN, RN) at Trilogy Health Services oversees the resident assessment and care planning process, ensuring compliance with federal and state regulations for MDS, quality of care, and Medicare/Medicaid reimbursement. This role requires a current LPN or RN license and CPR certification, with 3–5 years in MDS/assessment in long-term care.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• 3–5 years' experience in MDS/assessment in senior residential care or long-term care.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents' status and triggers.
• Evaluate condition and data to determine need for additional assessments or amended admission assessment.
• Prepare and electronically transmit timely reports to national Medicare/Medicaid databases.
• Develop a written plan of care for each resident with goals to address problems/needs.
• Provide information to residents/families on Medicare/Medicaid and financial assistance programs.
• Ensure MDS notes are descriptive of services and resident responses.
• Assist with completing the care plan portion of discharge plans and implement committee recommendations.
