Trilogy Health Services in Logansport, IN is seeking an MDS Coordinator (LPN or RN) to oversee resident assessments and care planning, ensuring compliance with federal and state regulations related to MDS, quality of care, and Medicare/Medicaid reimbursement.

You will conduct MDS assessments, prepare care plans, communicate with families about options, and support discharge planning while aligning with campus committees' recommendations. This role offers competitive pay and Thrive benefits.

Qualifications

• Must have current, valid LPN or RN license.
• Three to five years of MDS or assessment experience preferred.
• Current CPR certification required.

Responsibilities

• Perform and manage MDS assessments and triggers.
• Assess residents’ needs and amend admission assessments as needed.
• Transmit timely reports to Medicare/Medicaid databases.
• Develop preliminary and comprehensive care plans for each resident.
• Educate residents/families on Medicare/Medicaid options.
• Ensure MDS notes accurately reflect services and outcomes.
• Assist with discharge planning and committee recommendations.
