Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Preferred 3 to 5 years' experience in MDS or assessment role in senior living.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment for resident evaluations.
• Develop written plans of care for residents.
• Prepare and transmit reports to Medicare and Medicaid databases.
• Provide information to residents on financial assistance programs.
