The role emphasizes accurate MDS evaluations, care plan development, and communication with residents and families about benefits and options. Ideal candidates hold a current LPN or RN license and CPR certification, with 3–5 years in MDS or related assessments.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• 3–5 years’ experience in MDS or assessment in a senior residential care, healthcare, senior living or long-term care environment preferred.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents’ physical, psychological and functional status, including Care Area Assessments (CAA) and triggers.
• Evaluate each resident’s condition and data to determine need for special assessment activities or amendments to the admission assessment.
• Prepare and electronically transmit timely reports to Medicare/Medicaid databases.
• Develop a written plan of care for each resident identifying problems, needs and goals.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs.
• Ensure MDS notes are informative and descriptive of services and resident response.
• Assist with completing the care plan portion of discharge planning and implement committee recommendations.
