Trilogy Health Services, Liberty Township, OH, is seeking an MDS Coordinator (LPN, RN) to oversee resident assessments and care planning, ensuring compliance with federal and state regulations for MDS, quality of care, and Medicare/Medicaid reimbursement.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Three (3) to five (5) years’ experience in the MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment, preferred.
• Current, valid CPR certification required

Responsabilités

• Conduct and complete the MDS assessment to evaluate residents’ physical, psychological and functional status, including CAAs and triggers.
• Evaluate each resident’s condition to determine need for special assessment activities or amendment of the admission assessment.
• Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases.
• Develop a written plan of care (preliminary and comprehensive) for each resident identifying problems/needs and goals.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs.
• Ensure MDS notes are informative and descriptive of services and resident response.
• Assist with completing the care plan portion of discharge planning and implement recommendations from committees.
