Job summary

The Meadows of Delphos, a Trilogy Health Services community, is seeking an MDS Coordinator (LPN/RN) to oversee resident assessment and care planning while ensuring compliance with federal and state regulations related to MDS, quality of care, and Medicare/Medicaid reimbursement. Qualifications

• Must hold a current valid LPN or RN license in the state.
• 3–5 years’ experience in MDS or assessment in senior care or long-term care.
• Current CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents’ physical, psychological and functional status, including Care Area Assessments and triggers.
• Evaluate each resident’s condition and data to determine needs for special assessments or changes to the admission assessment.
• Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases.
• Develop a written plan of care for each resident that identifies problems and goals.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs.
• Ensure MDS notes describe services and residents’ responses.
• Assist with completing the care plan portion of the discharge plan and implement committee recommendations.
