Job summary

Trilogy Health Services is seeking an MDS Coordinator (LPN/RN) at Cypress Pointe Health Campus in Englewood, OH to oversee resident assessments and care planning while ensuring compliance with federal and state regulations. You will conduct MDS assessments, coordinate with care teams, and prepare reports for Medicare/Medicaid.

The role requires an active LPN/RN license and CPR certification, with 3–5 years of MDS experience in long-term care. Trilogy offers competitive pay and benefits.

Qualifications

• Current, valid state LPN or RN license.
• Three to five years in MDS or assessment roles in long-term care preferred.
• CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents' status and triggers.
• Evaluate residents to determine need for additional assessments or changes to admission assessments.
• Prepare and electronically transmit reports to Medicare/Medicaid databases.
• Develop written care plans (preliminary and comprehensive) for each resident.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs.
• Ensure MDS notes are informative and descriptive of services and responses.
• Assist with care plan portions of discharge planning and implement committee recommendations.
