Job summary

Trilogy Health Services, LLC is seeking an MDS Coordinator (RN) to oversee resident assessments and care planning, ensuring compliance with federal/state regulations and Medicare/Medicaid reimbursement.

The role requires current RN license and CPR certification, with 3–5 years in MDS/assessment within a long-term care setting. Strong attention to detail and collaboration with care teams are essential.

Qualifications

• 3–5 years experience in an MDS/assessment role in long-term care or senior living.
• Must maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment, including CAA triggers.
• Evaluate residents' condition to determine need for assessments or care plan amendments.
• Prepare and electronically transmit reports to national Medicare/Medicaid databases.
• Develop a written plan of care for each resident detailing problems, goals and interventions.
• Provide information to residents/families on Medicare/Medicaid and financial assistance.
• Ensure MDS notes describe services and residents' responses.
• Assist with discharge plan care plan and participate in committees as needed.
