Job summary

The Orchards in West Virginia seeks an experienced MDS Coordinator to manage the RAI process, MDS, PPS documentation, CAAs, and care plans. You will work under the Clinical Documentation Specialist to ensure compliance with Medicare, Medicaid, OBRA, and state regulations while maximizing reimbursement and supporting quality resident outcomes.

Qualifications

• Graduate of an accredited School of Nursing.
• Current LPN or RN license in the state.
• Minimum of one year nursing experience; MDS coordination preferred.
• Knowledge of Medicare, Medicaid, OBRA regulations, PPS, Case Mix, and Quality Measures preferred.
• Experience in long-term care, skilled nursing, or geriatric care preferred.
• Strong assessment, organizational, analytical, and time management skills.
• Excellent communication and leadership abilities with residents, families, physicians, and interdisciplinary team members.
• Proficiency with electronic medical records and Microsoft Office applications.
• Completion of all required annual state-mandated education and in-service training.

Responsibilities

• Coordinate MDS assessments, PPS documentation, CAAs, and care plans.
• Audit MDS, CAAs, and care plans for accuracy and regulatory compliance.
• Lead the interdisciplinary team in developing, revising, and implementing resident care plans.
• Ensure compliance with Medicare, Medicaid, OBRA, and state regulations while maximizing reimbursement.
• Provide education and training to nursing staff on MDS requirements and documentation standards.
• Communicate with physicians, therapists, residents, families, and team members regarding assessments and care planning.
