Job summary

The Orchards Michigan is seeking an MDS Coordinator to join our team in Harper Woods. You will complete MDS assessments, care plans, and related documentation for residents in a skilled nursing setting.

Responsibilities include initiating care plans, auditing records for accuracy, coordinating with meetings such as Medicare and QAPI, and protecting resident confidentiality while supporting a collaborative team.

Qualifications

• Valid RN or LPN license is required.
• Prior MDS experience is required.
• Long Term Care experience is required.
• Must be able to work 40 hours a week.
• Must be highly organized, professional and motivated.
• Solid computer skills and strong communication abilities.
• Excellent communication skills and team orientation.
• Friendly and a team worker.
• Knowledge of skilled nursing facility survey process and regulations is helpful.

Responsibilities

• Completing accurate MDS assessments and care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars timely.
• Maintaining and updating all care plans and assessments as required.
• Monitoring and auditing clinical records for accuracy and timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting confidentiality of resident and facility information at all times.
• Participates in meetings and committees (Medicare, QAPI, Weight Management, etc.).
