The Orchards is seeking an experienced MDS Coordinator to join our clinical team. You will coordinate the Resident Assessment Instrument (RAI) process, ensure accurate documentation, support individualized care planning, and help maximize appropriate reimbursement.

You’ll collaborate with nursing, therapy, physicians, residents, families and the interdisciplinary team to maintain high standards of resident-centered care.

Qualifications

• Requires RN licensure in state of practice or eligibility.
• Minimum of one year nursing experience; MDS coordination preferred.
• Knowledge of Medicare/Medicaid OBRA, PPS, Case Mix and Quality Measures is preferred.
• Excellent communication, leadership and teamwork skills.
• Ability to work independently while collaborating with interdisciplinary team.

Responsabilités

• Coordinate MDS assessments, PPS documentation, CAAs and care plans.
• Schedule assessment reference dates and deadlines to maintain compliance.
• Lead interdisciplinary team in developing and revising resident care plans.
• Monitor Medicare certifications, skilled nursing utilization and reimbursement.
• Audit MDS and care plans for accuracy and regulatory compliance.
• Oversee admission, discharge, re-entry and Medicare tracking docs.
• Promote Quality Measures and resident outcome improvements.
• Support restorative nursing programs with nursing leadership.
• Attend and lead Medicare, UR and care planning meetings.
• Educate staff on MDS requirements and documentation standards.
• Maintain accurate electronic medical records.
• Provide coverage and support for department operations.
