Job summary

The Orchards in Alliance, OH is seeking an MDS Coordinator to lead the RAI process, ensure Medicare/Medicaid compliance, and support quality outcomes for residents.

You will coordinate MDS assessments, PPS documentation, CAAs, and care plans, working with nursing leadership and the interdisciplinary team to maximize reimbursement and maintain high-quality care.

The role requires an RN or LPN with nursing experience, strong organization, communication, and a commitment to resident-centered care.

Qualifications

• Licensed nurse with state licensure (RN or LPN).
• Long-term care or skilled nursing experience preferred.
• Knowledge of Medicare/Medicaid, OBRA, PPS, Case Mix, and Quality Measures preferred.
• Strong assessment, organizational, analytical and time-management skills.
• Proficiency with electronic medical records and MS Office applications.

Responsibilities

• Coordinate MDS assessments, PPS documentation, CAAs, and care plans.
• Lead interdisciplinary care planning meetings and ensure regulatory compliance.
• Monitor certifications, reimbursement, and quality measures to optimize resident outcomes.
• Audit MDS, CAAs and care plans for accuracy and completeness.
• Provide education and training on MDS requirements and documentation standards.
