Job summary

Jobot is seeking an MDS Coordinator for a Long Term Care Facility in the United States. The role focuses on overseeing the MDS assessment process with collaboration across nursing, therapy, social services, dietary, and other interdisciplinary team members.

The ideal candidate has an active nursing license and MDS experience in long-term care, with strong knowledge of RAI, MDS 3.0, PDPM, and CMS guidelines. This on-site position offers comprehensive benefits and growth opportunities.

Qualifications

• Active nursing license or ability to obtain one.
• Previous MDS experience in a skilled nursing or long-term care setting.
• Strong understanding of RAI, MDS 3.0, CMS/Medicare & Medicaid guidelines, and PDPM.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes.
• Manage the RAI/MDS process in accordance with CMS and state regulations.
• Develop and maintain individualized resident care plans with the interdisciplinary team.
• Monitor assessment schedules and ensure timely documentation.
• Review clinical documentation for accuracy, completeness, and appropriate reimbursement.
• Support Medicare/Medicaid reimbursement processes, including PDPM.
• Participate in care plan meetings and communicate with residents, families, and the care team.
• Monitor resident condition changes and update assessments and care plans accordingly.
• Maintain compliance with federal, state, and facility standards.
• Assist with audits, surveys, and quality improvement initiatives.
• Serve as a resource to clinical staff regarding MDS documentation and regulatory requirements.
