Responsibilities

• Coordinate the development and completion of the resident assessment (MDS).
• Maintain and update written policies and procedures that implement MDS and care plan.
• Assist the resident in completing the care plan portion of the discharge plan.
• Develop procedures with the Director of Nursing to inform assessment team members of new admissions.
• Assist in scheduling the resident assessment and care plan meetings.
• Assist in determining appropriate treatment and activities based on medical and social history.
• Participate in development and implementation of resident assessments and care plans, including reviews.
