Job summary

Jobot is seeking an MDS Coordinator in Iowa to oversee and coordinate the MDS assessment process within a multidisciplinary care team. The role requires coordinating and completing MDS assessments on schedule and ensuring accuracy for compliant reimbursement.

The MDS Coordinator will work closely with nursing, therapy, social services, dietary, and other team members to develop individualized resident care plans and maintain regulatory compliance across CMS/state requirements.

Qualifications

• Active nursing license or ability to obtain one.
• Previous MDS experience in a skilled nursing or long-term care setting preferred.
• Strong understanding of the RAI process, MDS 3.0, Medicare/Medicaid guidelines, and PDPM.
• Strong organizational skills and attention to detail.
• Ability to manage deadlines and multiple assessments simultaneously.
• Excellent communication and interdisciplinary collaboration skills.
• Knowledge of long-term care regulations and clinical documentation requirements.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes.
• Manage the RAI/MDS process in accordance with CMS and state regulations.
• Develop and maintain individualized resident care plans in collaboration with the interdisciplinary team.
• Monitor assessment schedules and ensure timely completion of required documentation.
• Review clinical documentation for accuracy, completeness, and appropriate reimbursement.
• Support Medicare and Medicaid reimbursement processes, including PDPM.
• Participate in care plan meetings and communicate with residents, families, and members of the care team.
• Monitor changes in resident condition and ensure assessments and care plans are updated appropriately.
• Maintain compliance with federal, state, and facility standards.
• Assist with audits, surveys, and quality improvement initiatives.
• Serve as a resource to clinical staff regarding MDS documentation and regulatory requirements.
