Job summary

Majestic Care is seeking an MDS Coordinator (RN) to supervise and coordinate MDS and care plans for residents in the facility, ensuring timely and accurate documentation per state requirements.

The role requires an active RN or LPN license, MDS 3.0 knowledge, and strong collaboration with the care team. CPR certification must be active or obtainable within 30 days of hire. Join a team dedicated to compassionate, high-quality care.

Qualifications

• Must hold active RN or LPN license in the state of employment.
• Experience in Long Term Care preferred.
• Proficient computer skills and data entry.
• Knowledge of MDS 3.0 and related guidelines.

Responsibilities

• Coordinate the MDS and Care Plan as per facility policies.
• Inform care team when care plans are due and ensure completion.
• Develop or revise resident care plans quarterly or on change of condition.
• Manage PPS and State MDS assessments and ensure documentation is complete.
• Coordinate care plan meetings and involve the care team.
