Job summary

Copper Ridge Health & Assisted Living Center is seeking a skilled MDS Coordinator in a skilled nursing facility setting. The MDS nurse will collect and assess information for residents' health and well-being, manage care plans, and ensure proper documentation and reporting using PCC.

The ideal candidate is organized, interested in management positions, and committed to high standards. COVID vaccination or approved exemption is required; flexible shifts, PTO and competitive salary are offered.

Qualifications

Current Maryland RN license (or compact license).
Current Maryland LPN license (or compact license).
BLS/CPR certification.
Knowledge of MDS, nursing care methods, health and safety guidelines.
COVID-19 vaccination or approved exemption is required.

Responsibilities

Monitor residents' health and well-being through admission to discharge per MDS process.
Plan, document, and submit compiled information for the MDS.
Communicate with staff, residents and families to address concerns and reflect care plans.
Maintain healthcare records and PCC usage per best practices.
Investigate patient/family complaints and resolve them.
