Job summary

Copper Ridge Health & Assisted Living Center seeks an MDS Coordinator to lead MDS processes for residents in a skilled nursing facility. The role emphasizes organization, management potential, and maintaining high standards across documentation and care planning.

The incumbent will monitor health, plan care, and coordinate with staff, residents, and families while ensuring accurate, compliant records and timely submissions.

Qualifications

• Current Maryland RN license (or compact/multi-state).
• Current Maryland LPN license (or compact/multi-state).
• BLS/CPR certification.
• In-depth knowledge of MDS, nursing care methods, health and safety guidelines.
• COVID-19 vaccines or approved exemption required to work.

Responsibilities

• Monitor residents' health and well-being from admission through stay using the MDS process.
• Plan, document, and submit compiled information as part of care records.
• Communicate effectively with staff, residents, and families to address concerns.
• Ensure care plans reflect required care levels and reflect best practices.
• Maintain complete healthcare records and reports; investigate and resolve complaints.

MDS Coordinator

Copper Ridge Health & Assisted Living Center is looking for a nurse to fill a MDS role in a skilled nursing facility setting. The MDS nurse, collects and assesses information for the health and well-being of residents in Medicare or Medicaid certified nursing homes. Our ideal candidate is organized, interested in management positions, and committed to maintaining high standards. Must be COVID vaccinated, or have an approved vaccination exemption.

Duties

• Following federally mandated procedures and using the MDS process, the nurse monitors residents' health and well-being from the time of admission through the end of their stay.
• This work also requires the planning, documentation, and submission of the compiled information.
• MDS calls for creative problem solving and attentive communication with staff, residents, and their families, to ensure that personal, health and administrative concerns are being addressed. Ensure individual care plans reflect the type of care required.
• Maintain complete healthcare records and reports, including PCC being utilized according to best practices.
• Investigates patient/family complaints and takes appropriate actions to bring to resolution.

Requirements

• Current Maryland Registered Nursing Licensure (or compact/multi-state license)
• Current Maryland Licensed Practical Nursing Licensure (or compact/multi-state license)
• BLS/CPR certification
• In-depth knowledge of MDS, nursing care methods, health, and safety guidelines
• *Per the Centers for Medicare & Medicaid Services (CMS) guidelines, COVID-19 vaccines, or an approved religious/medical exemption, are mandatory and are a requirement to work in our community. Peak Healthcare does offer the COVID-19 vaccination to all employees at no cost.

Preferred Skills

• MDS certification by the American Association of Nurse Assessment Coordinators (AANAC)-preferred, not required.
• Experience with Point, Click, Care platform, Management experience
