Job summary

Halfway to Heaven Nursing Home is seeking a MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring that documentation complies with federal, state, and certification guidelines. The role coordinates the RAI process, MDS, CAAs, and care plan completion across disciplines to optimize care and reimbursement.

The candidate should be an RN or LVN licensed in the state, with at least 2 years of SNF nursing experience and strong knowledge of Case-Mix, Medicare PPS,

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA, and MDS processes.

Responsibilities

• Assist the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets guidelines.
• Coordinate the RAI process, ensuring timeliness and completeness of MDS, CAAs, and care plans.
• Participate in regulatory surveys and correction of deficiencies.
• Report audit trends to the Quality Assurance Committee.
• Monitor MDS process tracking and Medicaid tracking forms; ensure compliance with standards.
