Job summary

Opco-Skilled-Management- seeks an MDS Coordinator to assist the Director of Nursing and the RN Assessment Coordinator, ensuring documentation meets Federal, State, and Certification guidelines and coordinating the RAI process for timely MDS, CAAs, and care plans.

Requirements: RN/LVN license in the state; minimum 2 years in a Skilled Nursing Facility; strong knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA, and the MDS process.

Qualifications

• RN or LVN license in the state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility.
• Strong knowledge of OBRA, MDS, Case-Mix, Medicare PPS, and Medicaid reimbursement.

Responsibilities

• Assists the center in assuring adherence to Federal and State regulations and certification.
• Participates in regulatory surveys and assists in correcting deficiencies.
• Reports trends from audits to the Quality Assurance Committee.
• Ensures timely completion and submission of MDS, CAAs, and care plans.
• Monitors RAI process tracking, discharge/reentry and Medicaid tracking forms.
• Follows up with staff to ensure compliant documentation.
• Completes patient assessments, data collection, and interviews staff as needed.
• Facilitates accurate determination of the Assessment Reference Date.
