Job summary

SNF Staffing Solutions seeks a qualified MDS/Nursing Coordinator to manage the RAI process from admission to discharge in accordance with federal, state, and certification guidelines. You will work under the Director of Nursing and QC reimbursement leadership to coordinate MDS, care plans, and related documentation.

Responsibilities include interdisciplinary collaboration, MDS coding, data entry, and ensuring timely submissions to state databases; experience with PCC is preferred.

Qualifications

• Graduate of an approved Licensed Vocational program and licensed in the state of practice, required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required.
• Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on MDS/assessments follow clinical criteria and regulations.
• Coordinate daily stand-up meetings and review resident care and assessment reference dates.
• Comply with federal/state regulations regarding MDS processing and coordination.
• Monitor MDS and care plan documentation in medical records to support coding.
• Maintain current MDS status per state and federal guidelines.
• Enter resident information into MDS programs accurately.
• Complete MDS coding using chart reviews and staff/family interviews.
• Attend interdisciplinary meetings to gather information and communicate changes.
• Assist with significant change identification and physician orders to reflect MDS and care plan updates.
• Prepare scheduling and notices for care planning conferences and communicate outcomes.
• Keep knowledge base updated related to data entry and computer tech.
• Submit required documentation to state databases per policy.
• Ensure timely MDS submissions with validation follow-up.
• Participate in regulatory/certification surveys and deficiency corrections.
• Report trends from audits to QA Committee.
• Ensure RAI process completion from MDS to care plan.
• Initiate and monitor RAI tracking, discharge/reentry, and Medicaid forms in PCC.
