Job summary

Outfield Healthcare Partners is seeking an MDS Coordinator who will assist the Director of Nursing in ensuring documentation meets Federal, State, and Certification guidelines. The role coordinates the RAI process to ensure timely and complete MDS, CAAs, and the interdisciplinary care plan.

The successful candidate will have RN or LVN licensure with at least two years in a Skilled Nursing Facility, plus strong knowledge of Medicare PPS, Medicaid reimbursement, OBRA regulations, and care

Qualifications

• RN or LVN degree and state license required.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA, and data sets is essential.
• Understanding of care planning process and regulatory surveys is expected.

Responsibilities

• Assist in ensuring documentation complies with Federal, State, and Certification guidelines.
• Coordinate the RAI process, ensuring timeliness and completeness of MDS, CAAs, and Interdisciplinary Care Plan.
• Participate in regulatory surveys and assist in correcting deficiencies.
• Report trends from audits to the Quality Assurance Committee.
• Manage RAI process tracking, discharge/reentry, and Medicaid tracking forms.
