Job summary

Opco-Skilled-Management- seeks an MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets Federal, State, and Certification guidelines. The role coordinates the RAI process to ensure timely, complete MDS, CAAs, and the Interdisciplinary Care Plan.

The candidate will actively participate in surveys, track RAI activity, and manage Medicaid and discharge forms using PointClickCare, while maintaining validation records and ensuring accurate

Qualifications

• Knowledge of MDS/RAI process and care planning.
• Understanding of OBRA regulations and Minimum Data Set.
• Familiarity with Medicare PPS and Medicaid reimbursement.
• Regulatory compliance awareness.

Responsibilities

• Assures adherence to Federal and State regulations and certification.
• Participates in survey processes and corrects deficiencies.
• Reports trends from audits to the Quality Assurance Committee.
• Manages RAI process from MDS to interdisciplinary care plan.
• Monitors RAI process tracking, discharge/reentry, and Medicaid forms in PointClickCare.
• Follows up with staff to ensure documentation standards.
• Completes patient assessments and data collection per MDS User’s Manual.
• Determines Assessment Reference Date reflecting care needs and resources for Medicare/Medicaid.

Education

RN/LVN license in state of practice
