Job summary

Forest-Park-Nursing- is seeking an experienced MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets Federal, State, and Certification guidelines. This role coordinates the RAI process, ensuring timely and complete MDS, CAAs, and Interdisciplinary Care Plan.

You will oversee the MDS submission, track validation errors, and collaborate with staff to maintain accurate records and compliant care planning within a Skilled Nursing Facility

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Federal Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.

Responsibilities

• Assists the center in assuring adherence to Federal and State regulations and certification.
• Actively participates in the regulatory or certification survey process and the correction of deficiencies.
• Reports trends from completed audits to the Quality Assurance Committee.
• Assures the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms through the PointClickCare system.
• Follows up with staff to assure compliance to standards of documentation.
• Completes patient assessments, data collection, and interviews staff as necessary to assure good standard of practice.
• Facilitates accurate determination of the Assessment Reference Date reflecting patient care needs for appropriate payment.
• Ensures timely submission of the MDSs to the State with proper follow-up on validation errors.
