Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator for Forest Park Nursing Center. You will ensure timely and accurate completion of the RAI and care management processes from admission to discharge in line with policy, federal and state guidelines.

Reporting to the Director of Nursing and VP of Clinical Reimbursement, you will coordinate data entry into MDS programs, support the interdisciplinary team, and maintain compliant documentation.

Qualifications

• RN or LVN with Texas license.
• Experience with MDS 3.0 and care planning.

Responsibilities

• Oversee timely completion of the RAI and care management from admission to discharge.
• Coordinate interdisciplinary team for MDS assessments and care plan reviews.
• Ensure compliance with federal/state regulations and OBRA guidelines.
• Maintain accurate MDS data entry and documentation in medical records.
• Complete MDS coding using chart review, observation and staff interviews.
