Gallup Nursing & Rehabilitation is seeking an experienced MDS Coordinator (RN) to ensure timely completion of the RAI/MDS cycle from admission to discharge in compliance with federal, state, and facility policies.

You will collaborate with the interdisciplinary team to schedule assessments, review care plans, and ensure accurate coding and documentation in the medical record. This role requires strong knowledge of MDS 3.0, Medicare PPS, and Medicaid reimbursement.

Qualifications

• MDS/RAI processes and coding experience.
• Understanding of the MDS 3.0 data elements and documentation requirements.
• Familiarity with federal/state regulations related to MDS and care planning.
• Experience with care plan reviews and interdisciplinary team meetings.

Responsabilités

• Coordinate MDS assessments and care plan reviews with the interdisciplinary team.
• Ensure timely and accurate completion of MDS and RAI submissions.
• Monitor documentation in medical records to support MDS coding.
• Assist with shifts in scheduling and communications regarding resident care planning.
• Maintain confidentiality and comply with regulatory requirements.
