Job summary

Gallup Nursing & Rehabilitation is seeking an MDS Coordinator to manage timely and accurate completion of RAI and care management from admission to discharge in line with company policy and federal/state guidelines. You will coordinate information systems and educate the clinical team under the direction of nursing leadership.

You will collaborate with the Interdisciplinary Team, ensure proper MDS coding and documentation, and maintain up-to-date knowledge of MDS processes and regulatory

Qualifications

• RN license in state of practice
• Minimum 2 years of nursing experience in a Skilled Nursing Facility
• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement
• Understanding of OBRA regulations and Minimum Data Set (MDS)
• Knowledge of the care planning process
• Experience with MDS 3.0 (preferred)

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Coordinate decisions on MDS completion based on clinical criteria per the RAI User’s Manual.
• Assist with coordination of daily stand-up meetings and assessment reference dates.
• Ensure compliance with federal and state regulations for RAI process completion.
• Monitor MDS and care plan documentation in medical records to support coding.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into computerized MDS programs accurately.
• Code MDS information from medical records, observations, and staff input.
• Attend interdisciplinary meetings to gather information and update records.
• Assist with identification of significant changes and translate into revised MDS and care plans.
• Schedule resident care planning conferences and communicate outcomes to staff and families.
• Continuously update knowledge related to data entry and computing technologies.
• Submit required documentation to state databases per policy.
• Ensure final MDS accuracy and proper submission to state/federal agencies.
• Instruct staff on RAI process, PPS Medicare, Medicaid, and clinical systems.
• Maintain confidentiality of information.
