Job summary

Gallup Nursing & Rehabilitation is seeking a MDS Coordinator (RN) to ensure timely completion of the RAI/MDS processes from admission to discharge in accordance with policy, procedures, and regulatory guidelines. The role involves collaborating with the interdisciplinary team, coordinating assessments, and ensuring accurate MDS coding and documentation.

The ideal candidate will have RN licensure, experience with MDS 3.0, and strong knowledge of Medicare PPS and Medicaid reimbursement.

Qualifications

• RN license (state where practicing)
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement and OBRA regulations
• Experience with MDS 3.0 is preferred

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Decide on MDS completion based on clinical criteria and MDS User’s Manual.
• Assist with daily stand-up meetings and setting assessment reference dates.
• Ensure compliance with federal and state regulations in MDS processes.
• Monitor and document MDS status and data entry in state databases and EMR systems.
• Attend interdisciplinary meetings to communicate changes and update records.
• Assist DON with significant changes, physician orders, and verbal reports.
• Prepare scheduling and notices for care planning conferences.
