Job summary

Casa Maria Healthcare is seeking an MDS Coordinator to ensure timely and accurate completion of RAI and care management processes from admission to discharge. The role operates under the Director of Nursing and VP of Clinical Reimbursement and involves coordinating information systems and staff education in the clinical department.

Qualifications

• RN or LVN license in the state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Familiarity with OBRA regulations and the MDS process.
• Experience with MDS 3.0 preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure timely, accurate completion of RAI processes from admission to discharge.
• Monitor MDS and care plan documentation in the medical record.
• Maintain up-to-date MDS status per state and federal guidelines.
• Assist with daily stand-up meetings and data entry into MDS programs.
