Qualifications

• Bachelor’s in Nursing required and RN license active in the state of employment.
• Proficiency in MDS 3.0 and understanding of state/federal regulations.
• Experience coordinating MDS, CAAs and care plans in LTC settings.

Responsibilities

• Oversee accurate completion of MDS, CAAs and Care Plans per federal/state regulations.
• Communicate with IDT, administrator and family to identify clinical needs and discharge barriers.
• Ensure timely electronic submission of MDS data to state databases and address validation reports.
