Job summary

OPCO Skilled Management in Mineola, TX seeks an experienced MDS Coordinator to manage RAI and care planning from admission to discharge, under Nursing leadership and VP Clinical Reimbursement. This RN role ensures compliance with federal/state guidelines and maintains accurate MDS coding.

The role requires coordinating the daily stand-up for resident care, setting assessment reference dates, and ensuring all MDS documentation is accurate in the medical record to support proper coding.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required.
• Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing or not completing additional MDS based on clinical criteria per the RAI User’s Manual.
• Coordinate daily stand-up meetings reviewing resident care and assessment reference dates.
• Ensure compliance with federal and state regulations governing the RAI process.
• Monitor MDS and care plan documentation in the medical records and ensure accuracy for coding.
• Maintain current MDS status per state and federal guidelines.
• Maintain data entry into appropriate computerized MDS programs.
• Code the MDS accurately from medical records and staff/family input.
• Attend interdisciplinary team meetings to gather information and update records.
• Assist DON with identification of significant changes and communication of outcomes.
• Prepare scheduling and notices for care planning conferences and communicate outcomes.
• Update knowledge base related to data entry and computer technology.
• Submit electronic documentation to state databases per policy.
• Ensure final MDS completion and submission to authorities.
• Assign and train staff in the RAI Process, PPS Medicare, Medicaid and clinical systems.
• Maintain confidentiality of information.
• Other duties may be assigned.
