Job summary

Socorro Wellness & Rehabilitation is seeking an experienced MDS Coordinator to lead timely RAI and care management from admission to discharge. You will work with the Director of Nursing and VP of Clinical Reimbursement to maintain compliance with federal and state guidelines and to ensure accurate MDS coding and care plans.

Responsibilities include coordinating MDS activities, attending interdisciplinary meetings, and ensuring timely data entry into the facility’s systems.

Qualifications

• RN/LVN license in the applicable state.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility.
• Thorough knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Knowledge of the MDS 3.0 and OBRA regulations preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on whether to pursue additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings to review care and set assessment reference dates.
• Comply with federal and state regulations for MDS processing and care planning.
• Monitors MDS and care plan documentation for all residents, ensuring accuracy in medical records.
• Maintain current MDS status per state and federal guidelines and input data into MDS software.
• Assist DON or designee with significant changes, physician orders, and verbal reports to reflect changes in MDS and care plans.
• Attend weekly Medicare, Medicaid and Managed Care meetings as required.
