Job summary

US Skilled Serve is seeking an MDS Nurse Coordinator to lead and coordinate the resident assessment process in accordance with state requirements. The role includes administering and coordinating RAPs, Triggers, and care plans in collaboration with the care team.

The incumbent will ensure timely and accurate MDS submissions, support discharge planning, and maintain alignment with residents’ wishes and regulatory standards.

Qualifications

• Must understand MDS processes and RAPs/Triggers.
• Familiar with regulatory requirements governing resident assessments.
• Ability to ensure accuracy and completeness of assessment data.
• Strong communication with health professionals and care teams.

Responsibilities

• Coordinate the development and completion of resident assessments (MDS) per state rules and facility policies.
• Perform administrative duties such as data entry, forms, reports, and evaluations.
• Implement and maintain a quality assurance program for assessments and care plans.
• Coordinate with Discharge Planning to complete care plans for residents.
• Ensure timely submission of assessments and report issues to administration.
• Maintain up-to-date MDS Instructor's Manual for staff.
• Foster effective relationships with health professionals, physicians, and government agencies.
• Meet with department supervisors to discuss assessment-related functions.
• Schedule and conduct resident interviews for assessments.
• Evaluate residents’ conditions to determine need for special assessments or plan amendments.
• Assist directors and supervisors in scheduling assessment meetings.
• Coordinate scheduling with care plan team members and family representatives.
• Ensure care provided aligns with residents' wishes.
