Job summary

Journey is seeking an MDS Coordinator to participate in the Resident Assessment Process by completing MDS items and CAAs, and to help develop individualized resident care plans in line with federal and state regulations.

The role requires a current RN license in the state of employment and strong knowledge of MDS 3.0, with collaboration across multidisciplinary teams to drive improved resident outcomes.

Qualifications

• Must hold a current nursing license in the state of employment.
• Working knowledge of the MDS 3.0 (current version of minimum data set).

Responsibilities

• Conduct and coordinate MDS assessments and CAAs, develop individualized care plans, and update as residents’ conditions change.
• Ensure accurate completion and submission of MDS assessments per Medicare/Medicaid guidelines and state/federal regulations.
• Facilitate care plan meetings and maintain effective communication across departments to improve resident care outcomes.
