The Nurse Assessment Coordinator oversees the MDS assessments and care planning for residents, ensuring compliance with federal and state guidelines and accurate documentation to support quality care and reimbursement.

You will collaborate with interdisciplinary teams, educate staff on guidelines, participate in care plan meetings, audits, and surveys, and contribute to data analysis and quality assurance.

Responsibilities

• Coordinate and complete accurate and timely MDS assessments and care plans for all residents.
• Ensure compliance with all federal, state, and company regulations and guidelines.
• Work collaboratively with interdisciplinary teams to gather information and develop individualized care plans.
• Monitor and analyze data for trends, accuracy, and quality assurance.
• Participate in care plan meetings, audits, and surveys as needed.
• Educate staff on documentation best practices and changes to MDS guidelines.
